What is microneedling?
Microneedling — also known as Collagen Induction Therapy (CIT) or skin needling — is a minimally invasive aesthetic medicine treatment that exploits the skin's natural regenerative capacity. Using a device fitted with sterile precision needles, controlled micro-injuries are created in the superficial and mid layers of the dermis, triggering an endogenous repair response.
This response releases growth factors and stimulates the synthesis of new type I and III collagen, along with elastin and endogenous hyaluronic acid. The result is skin that becomes progressively denser, more elastic, firmer and more luminous, with visibly improved texture.
Unlike ablative lasers, microneedling does not remove layers of epidermis: the skin barrier remains intact, making the treatment safe even for darker phototypes (IV–VI), with a negligible risk of post-inflammatory hyperpigmentation.
Main indications
- Superficial wrinkles and fine lines — forehead, eye contour, lip contour
- Acne scars — atrophic, ice pick, boxcar and rolling scars
- Enlarged pores — visible reduction of pore diameter
- Stretch marks — white and recent, on abdomen, thighs, breast and buttocks
- Hyperpigmentation and spots — melasma, sun spots, discolouration
- Loss of tone and radiance — dull skin or signs of photoageing
- Androgenetic alopecia — combined with specific serums to stimulate the hair follicle
How the treatment is performed
The microneedling protocol followed by Dr. Marco Galati is a standardised, safe procedure adapted to each patient's individual needs:
- 1. Consultation and skin assessment — before starting the cycle, Dr. Galati performs an in-depth analysis of skin type, treatment areas and goals. Any contraindications (active acne, skin infections, coagulopathies, pregnancy) are evaluated, and the personalised plan is defined: number of sessions, needle depth and any boosting serums.
- 2. Skin preparation — the skin is thoroughly cleansed of make-up, sebum and impurities. A lidocaine-based topical anaesthetic cream is applied for 30–45 minutes for maximum comfort, then removed before disinfection.
- 3. The needling itself — the physician uses a medical pen device (electric pen or dermapen) with sterile single-use needles between 0.25 and 2.5 mm. Penetration depth is calibrated to the treated zone and the indication: more superficial for radiance and pores, deeper for scars. The device is moved systematically across the whole area to guarantee uniform distribution of the micro-channels.
- 4. Application of active serums (optional) — immediately after the micro-channels are created, the skin can absorb active ingredients up to 80 times more effectively than with ordinary topical application. This is the ideal moment to deliver boosting serums such as Platelet-Rich Plasma (PRP), cross-linked or non-cross-linked hyaluronic acid, stabilised vitamin C, collagen peptides or amino acid cocktails, according to the therapeutic goals.
- 5. Immediate post-treatment care — a soothing, hydrating mask is applied to calm the redness and support the skin barrier. The physician provides precise home-care instructions: no make-up for 24 hours, gentle cleansers and non-occlusive moisturisers only, SPF 50+ every morning for at least two weeks.
Recovery and results
Microneedling has minimal social downtime. In the hours after the session the skin appears reddened, similar to mild sunburn, with possible slight swelling and warmth. These reactions are entirely normal and resolve spontaneously within 24–72 hours.
Recovery timeline
- Hours 0–24: redness and warmth. Avoid make-up, sauna, intense sport and alcohol.
- Days 2–3: the skin may feel slightly dry with fine flaking. Moisturise generously.
- Days 4–7: the skin returns to its normal appearance, with a first visible improvement in radiance.
- Weeks 2–4: collagen synthesis is in full activity; texture improves progressively.
- Months 2–3: peak results. Scars, pores and superficial wrinkles appear visibly reduced.
Long-lasting results
The results of microneedling are not as immediate as those of a filler, but they are more natural and durable because they rely on the skin's intrinsic regeneration. A complete cycle of 4–6 sessions delivers results that last 12–18 months. To preserve and amplify the benefits, a maintenance session every 6–12 months and a home routine with SPF and antioxidant actives are recommended.
Synergistic combinations
Microneedling combines well with other aesthetic medicine treatments for more complete results. Dr. Galati may suggest integrating botulinum toxin for dynamic wrinkles, chemical peels for pigmentation, or hyaluronic acid fillers for volume restoration. Combined protocols are always planned individually at the initial consultation.
Frequently asked questions
The treatment is generally well tolerated. A topical anaesthetic cream is applied for about 30–45 minutes before the session, making it essentially painless. During the procedure a slight warmth or tingling may be felt. Afterwards the skin looks reddened, like moderate sun exposure, but the discomfort is minimal and resolves within 24–72 hours without painkillers.
For most indications — fine lines, enlarged pores and general radiance — 3–4 sessions at 4–6 week intervals are recommended. Deeper acne scars or stretch marks may need 4–6 sessions. After the initial cycle, a maintenance session every 6–12 months preserves the benefits over time.
First improvements in radiance and complexion appear 1–2 weeks after the first session. The most significant results — particularly for scars and wrinkles — appear after 4–8 weeks, when new collagen synthesis is fully under way, peaking at about 3 months after the last treatment and lasting 12–18 months. The gradual progression guarantees natural-looking results.
Microneedling is indicated for all phototypes, including darker skin (IV–VI), unlike some lasers that carry a risk of post-inflammatory hyperpigmentation. It is not indicated with active skin infections, acute inflammatory acne, severe rosacea, psoriasis or eczema in the treatment areas, or during pregnancy and breastfeeding. Oral isotretinoin requires a suspension of at least 6 months. A preliminary consultation with Dr. Galati assesses individual suitability.
Yes. Immediately after the micro-channels are created, the skin absorbs active ingredients far more effectively, so boosting serums such as PRP, hyaluronic acid or vitamin C can be applied during the session. Microneedling also pairs well with botulinum toxin, chemical peels and fillers in an integrated treatment plan.
